Beyond the mouth of the Altamaha River are the coastal barrier islands of Sapelo, to the north, and St. Simons, to the south. These and the other barrier islands along Georgia's coast provide a protective barrier between land and sea.

In the delta near the mouth of the river are several islands formed from sediment carried down the river from eroded areas upstream.
